REQUEST FOR MVD HEARING

Warning! Unless you make a written request within ten (10) days of your arrest for DUI, your privilege to drive will be revoked twenty days after your arrest. This is true if:

(a) you have been charged with refusal to submit to a breath or blood test;

(b) you are over 21 years of age with a blood alcohol content of .08 or higher; or,

(c) you are under 21 with a blood alcohol content of .02 or higher.
