The New Mexico Motor Vehicle Deparment must receive your WRITTEN request for a hearing on the revocation of your Driver's License within ten (10) days from the date you were served with a Notice of Revocation. Usually this means within ten (10) days of your arrest because, in most cases, the arresting officer will serve you with a Notice of Revocation form. In rare cases, the notice will be mailed to you via registered mail.
To request a hearing, you must state in writing your desire for a hearing concerning the revocation of your license and either attach a photocopy of the front of the Notice of Revocation or include complete identifying information, including your name, date of birth, social security number, driver's license number, address, phone number, citation number, date of arrest and the name of the law enforcement agency involved in your arrest.

Your request may be submitted in person at: Driver Services Bureau of the Motor Vehicle Division The Montoya Building, Roome 2093 1100 S. St Francis Drive Santa Fe, New Mexico. The DUI office, room 2093, is located on the second floor in the Southeast Corner of the atrium.
